Request a Service, Product or Personal Support
One application. One Counsellor. Access to the wider Dignified Independence service ecosystem.
You do not need to know the name of the correct department, service, specialist or programme before contacting Dignified Independence. Tell us what you need, where you are, and how we can best communicate with you — your Counsellor will help identify the right pathway.
Who It's For
Any person who needs an applicable DI service, product, personal support or professional pathway.
How It Starts
One Service Request Application connects you with a Counsellor who navigates the DI system on your behalf.

Open to everyone with an applicable need
Any person who needs an applicable Dignified Independence service, product, personal support or professional pathway — including a person with a disability, an older person, a person living with illness or long-term dependency, a family member or caregiver, or a member of the general public.
A Single or Several Connected Needs
Whether you are a person with a disability, an older person, a person living with an illness or long-term dependency, a family member or caregiver, or a member of the general public, you may use this application to tell us what you need, where you are, and how we can best communicate with you.
Your requirement may relate to a single product or service, or several connected needs. Examples include:
You Do Not Have To Identify The Right Department
Tell us the actual problem, requirement or outcome you are seeking. Your Counsellor will help identify the appropriate pathway.
Missing Or Unavailable Documents
If any mandatory document or certificate is unavailable, not applicable, under process, or cannot be submitted for any genuine reason, upload a signed declaration on a plain white sheet stating the name of the missing document and the reason for its non-submission. This declaration may be uploaded in place of the document to complete the application; however, the original or additional supporting documents may be requested later if required.
The Counsellor is the first professional navigation point. The Counsellor understands the person and the wider DI system. Once the correct pathway is identified, the relevant Service Secretary or department handles the service-specific process, and the appropriately qualified Specialist or service team delivers or coordinates the approved intervention.

Counsellor Matching
Wherever an appropriate match is available, DI aims to connect you with a Counsellor familiar with your locality or region. If an exact local match is unavailable, matching can consider a nearby culturally similar region, the same language and cultural background, and then the closest appropriate available Counsellor.
Free Initial Feasibility
Every new service request proceeds through the mandatory free Trieye feasibility assessment before approval or activation. Submission of this form begins the authorised process; it does not itself constitute service approval.
Complete The Service Request Application Below
Fields marked required are needed to begin. If you are unsure of an answer, complete only what you know at this stage — your Counsellor can help clarify the rest. There is no fee to submit this form.
What Happens Next
Your application enters the authorised Dignified Independence initial process. For a properly submitted new application, the established service framework aims to complete the initial review, identify the closest appropriate available Counsellor and connect the applicant with that Counsellor within a maximum of 72 hours.
The Counsellor Understands You First
You do not need to navigate the complete Dignified Independence service structure yourself. Your Counsellor helps clarify the actual requirement, understand relevant personal and regional circumstances, and identify the appropriate pathway.
Emergency Notice
Submitting this application does not replace emergency medical, police, fire or other statutory emergency services. In an emergency, contact your local emergency services directly.
The Same Ten Steps, Now In Motion
Before You Move On
- This is the general Service Help Desk application for a person seeking an applicable DI service, product, support or professional pathway.
- Employment applicants and entrepreneurs/business applicants may use their dedicated application pathways where those are more appropriate to the primary request.
- A service request may involve one service or several connected DI capabilities.
- The absence of disability, old age or illness does not by itself exclude a person from requesting an applicable DI service.
- All requests remain subject to feasibility, applicable law, professional requirements, service availability, capacity and the terms of any approved service agreement.
