Restricted Eligibility — Please Read First
This is not a public opinion, general commentary, anonymous allegation, media-query or open public complaint channel. This form only becomes usable once a valid Dignified Independence enquiry, application, service, customer, transaction, employee, training, programme, case or other recognised reference is verified. If you have never used, received, applied for, enquired about, purchased, participated in, or otherwise formally interacted with a DI product, service, programme or process, please use our Contact Us page instead.
Mandatory Upload Instruction
If any mandatory document or certificate is unavailable, not applicable, under process, or cannot be submitted for any genuine reason, upload a signed declaration on a plain white sheet stating the name of the missing document and the reason for its non-submission. This declaration may be uploaded in place of the document to complete the form; however, the original or additional supporting documents may be requested later if required.

An Enquiry, Application, Customer/Service, Transaction/Receipt/Invoice, Employee/Appointment, Training/Course, Programme/Case, or Contract/Partner reference number.
Matching Registration Details
The name, email address, mobile/WhatsApp number and country connected to the original enquiry, application or service.
Approximate Original Date
Roughly when the original interaction took place, and a short description of what it was about.

What Happens Next
Your complaint is registered and a unique complaint reference number is generated. Depending on the case-update preferences you selected, you may be notified when the complaint is formally registered, when initial screening completes, if it is referred for specialised review, when further information is requested from you, and when a final decision is reached.

Your reference is checked, a complaint number is issued, and you receive confirmation of the submission date and time.
Initial Screening
The complaint is reviewed for scope and, where relevant, routed to the appropriate specialised section or investigation unit.
Review & Clarification
Relevant persons or entities may be given an opportunity to respond. You may be asked for clarification, further evidence or a statement.
Outcome
You are notified of the outcome according to your selected update preference, with a downloadable or emailed copy available on request.
A Complaint Is Not A Finding

Complaints are not treated as proven merely because they were submitted — allegation, evidence, response, findings and outcome are kept clearly separate throughout the review.

Confidentiality Handling
Choose normal handling, limited disclosure where reasonably possible, or highly sensitive review before any disclosure — and identify anyone who should not be informed of your identity unless reasonably necessary.
